500+ Families Served

A temporary illness or long-term diagnosis can quickly turn everyday routines into challenges. Whether a loved one is recovering from cardiac surgery, managing a Parkinson's diagnosis, or navigating cancer treatment, the demands on daily life can be significant. At Passion for Seniors of NY, we provide compassionate, consistent in-home care tailored to each person's condition and schedule, throughout New York City and Nassau County.

Illness affects more than physical health. It can disrupt routines, strain family relationships, and erode a sense of independence. Our caregivers understand that. We build a care plan around your loved one's life, not a template.

Every care relationship begins with a nurse-led assessment designed to understand medical needs, daily routines, preferences, and family concerns. Working with your physician and household, our Registered Nurse develops a personalized Plan of Care that guides your caregiver's activities from day one.

  • RN visits your home on day one, accompanied by your caregiver, for a full clinical assessment.

  • A personalized Plan of Care is developed with input from your physician and family.

  • Care is available hourly, daily, or live-in — minimum 4 hours for hourly service.

  • Nursing Supervisors available on-call 24/7 for urgent needs.

Services May Include

  • Medication reminders based on prescribed schedules

  • Assistance with personal care and hygiene

  • Emotional support and companionship

  • Monitoring for changes in symptoms or health status

  • Light meal preparation based on dietary guidelines

  • Coordination with your physician and care team

  • Transportation to medical appointments

Our caregivers are thoroughly screened, bonded, and insured through the agency, and every placement includes credential verification, criminal background checks, and a current physical exam. Whether your family needs a few hours of daily support or full-time live-in care, we can accommodate your schedule and your needs.
