At Passion for Seniors of NY, we provide dependable, professional in-home convalescence support across all five NYC boroughs and Nassau County, helping seniors recover safely during an important stage of healing.

The period immediately following discharge is when the risk of readmission is highest. Missed medications, limited mobility, and inadequate support at home are among the leading reasons seniors return to the hospital within 30 days. Our caregivers are trained to recognize and address these risks before they become problems.

On the first day of service, our Registered Nurse visits the home alongside your caregiver to conduct a thorough clinical and home assessment. Working with your discharge team, physician, and family, the nurse establishes a personalized Plan of Care that guides everything from medication reminders to daily activity. That plan evolves as recovery progresses.

How Care Works

  • RN visits your home on day one, accompanied by your caregiver, for a full clinical assessment.

  • A personalized Plan of Care is developed with input from your physician and family.

  • Care is available hourly, daily, or live-in — minimum 4 hours for hourly service.

  • Nursing Supervisors available on-call 24/7 for urgent needs.
