Every client begins with a home visit from one of our licensed Registered Nurses. This isn’t a quick stop. It’s a comprehensive assessment that looks at medical needs, mobility, daily routines, the home layout, and family involvement.
From there, we build a custom Plan of Care, which may include:
Medication reminders and supervision
Nutrition and hydration guidance
Physical or occupational therapy coordination
Help with bathing, dressing, and getting around safely
Companionship and emotional well-being goals
We update the care plan as needed, based on progress and feedback. Nurses follow up regularly, and families stay fully in the loop.
The Critical Window After Discharge
The days and weeks after a hospital or rehab discharge can be risky. Falls, medication errors, or missed follow-ups can lead to setbacks or even readmission.
